Does insurance cover functional medicine visits at Forum Health Las Colinas?
Our providers are out-of-network. PPO plans may reimburse a portion of visit costs depending on your plan, and HMO plans typically don’t cover office visits but often cover lab work. Medicare cannot be used. We can provide documentation for insurance reimbursement submissions upon request.
