What is Bioidentical Hormone Replacement Therapy?
Bioidentical hormones are typically plant-derived substances that are an exact replica of hormones produced in the body. In essence, they have identical chemical structures.
Although both are created in labs, the difference between bioidentical hormones and synthetic hormones is that synthetic ones are not identical to the hormones created in your body. BHRT uses these natural hormones to restore levels and optimize health and wellness.

Bioidentical Hormones and Anti-Aging Services
The process of aging causes all individuals, regardless of sex, to experience varied and unique symptoms as their hormone levels change over time.
Perimenopause (the transition from childbearing years to menopause) and menopause usually occur gradually. However, if a woman undergoes a hysterectomy at any age, symptoms can come on suddenly.
Even if the ovaries are not removed, their function can still decrease because removal of the fallopian tubes can disrupt their blood supply. If the ovaries are removed as well, menopausal symptoms can be much more severe.

Dr. Kyler Elwell
Dr. Kyler Elwell-Silver is a board-certified physician passionate about medicine and deeply committed to patient care. After years of practicing conventional medicine in a successful career as an obstetrician-gynecologist, Dr. Elwell reached a point where she felt the need to explore a different approach and decided to transition to functional medicine. The increasing number of inquiries about functional medicine and the desire to provide comprehensive care beyond the limitations of 15-minute appointments prompted her to do some soul-searching and make a change.
Dr. Elwell found that patients wanted to understand the reasons behind their illnesses and desired solutions beyond symptom management. They sought answers to the “why” behind their health issues and longed for sustainable solutions. Inspired by this desire for comprehensive care, she embarked on a new path in functional medicine, eager to provide patients with the in-depth understanding and personalized treatment plans they desired.
Dr. Elwell completed her undergraduate and master’s degrees at the University of Nevada, Las Vegas, before pursuing her medical degree at the University of Texas Medical Branch in Galveston. She then completed her residency in obstetrics and gynecology at UT Southwestern, solidifying her foundation in women’s health. Dr. Elwell most recently became board certified in obesity medicine, further enhancing her ability to address the complex relationship between hormones, metabolism, and overall health.
Beyond her medical expertise, Dr. Elwell’s genuine care for her patients sets her apart. She values building strong relationships and being a trusted partner in her patients’ healthcare journeys. Patient care played a significant role in Dr. Elwell’s decision to focus on functional medicine. She realized that her favorite part of being a physician was the deep connections she formed with her patients. Physician Assistant Jennifer Mata
Jennifer Mata is a highly dedicated and compassionate Physician Assistant at Forum Health. She specializes in holistic and preventative care and has extensive skills and experience in Bioidentical Hormone Replacement Therapy, Sleep Care Management, Weight Loss Management, and Primary Care.
With a philosophy centered on natural approaches and patient education, Jennifer creates an open dialogue with her patients to ensure they are active participants in their health journey.
Her professional journey includes significant roles at various healthcare institutions and Medical Centers.
Jennifer’s commitment to her patients is deeply personal. She was a patient here for over a year, looking for answers to some of her health issues. “I started realizing quickly that what I was promoting with my weight management patients and hormone patients was what she was advocating,” Jennifer says. “I was doing something similar. That prompted me to want to learn more.” This journey led her to pursue a career in functional medicine here at Forum Health.
Jennifer’s approach begins with really listening to her patients to gain an understanding of what they have been experiencing and forming a partnership with every patient. Nurse Practitioner Debbie Bluestein
Debbie is passionate about helping people feel their best and achieve optimal health. She works with both women and men using Bioidentical Hormones and specializes in preventing illness and disease.
Debbie believes in using a very individualized approach to achieve optimal hormone balance for each patient. She also always listens closely to her patients and involves them in decision-making regarding the plan for their bodies and health. Debbie also collaborates with the practice in planning out a comprehensive program for clients.
She has been a Nurse Practitioner since 1998 and has had opportunities for extensive study and training in Bioidentical Hormone Replacement Therapy and optimization. She uses a functional medicine approach to anti-aging, using hormones, diet, exercise, nutraceuticals, and supplements.
Debbie worked in Family Practice during the early part of her career. She has been working with Bioidentical Hormone Replacement Therapy since 2011.
Physician Assistant Kam Neal
Growing up in Colorado, she remembers being fascinated with investigating how things worked. Kam attended Southern Nazarene University in Bethany, Oklahoma, where she received a bachelor of science in biology. She also completed the physician associate program and earned a master’s degree in health services from the University of Oklahoma Health Sciences Center.
She is a certified Physician Assistant and licensed by the Texas Medical Board.
Previously, in the traditional medical model, Kam felt frustrated working in a high-volume practice with barely enough time to refill medications and then not seeing her patients feel any better. In addition, her own experience with hormone imbalances after having children and being told “everything is normal” fueled those feelings. This led her to functional medicine and an optimal wellness model.
Kam’s desire to treat the whole patient and not just the symptoms, along with her passion for teaching, is evident: She loves working with each patient to find the root cause of their illness while teaching them about their health, so they feel informed and empowered.
Insurance Coverage
In order to delve more deeply and locate the underlying causes of conditions and provide optimal treatment, we find we need much more time with our patients and more treatment options than insurance companies will work with, for instance. Thus we are out of network for insurance.
However, if you have a PPO type of insurance plan, it is possible that your insurance will reimburse you for some or all of your office visit costs. Minimally, the costs should be able to go toward your deductible.
You will pay for the appointments at the time of service, and we can provide you with the papers to file for reimbursement with your insurance provider.
If you are on an HMO plan, your insurance will most likely not cover any of the costs of your office visits but may cover all or a majority of your lab work.
Regarding the comprehensive lab work that will be required, many insurance plans will cover your labs. However, we recommend you check with your insurance company to make sure. Some plans have changed how they pay for lab work and may require you to meet your deductible before your insurance pays anything. This could mean a significant increase in your cost of lab work.
In order to better serve our patients, we can offer you a significant discount through Quest Diagnostics or LabCorp for self-pay. We can also help you arrange to have a blood draw done at your home for a nominal fee.
