You’ll likely speak with our New Patient Coordinator when you first contact our office. They can take all the time you need to answer any questions, review our protocols and fees, introduce you to our providers, and help you determine if our practice fits your needs. If you choose to proceed, they can also schedule your new patient appointment for a day and time that suits you.

When you begin as a new patient, you will start your treatment plan with a 2-hour appointment. Future appointments are 1 hour long. You can book with Dr. Elwell or one of our Physician Assistants or Nurse Practitioner.
Information We Will Need
Once your new appointment is booked, you will receive a link to our Patient Portal and several consent forms. In this portal will be your Health Questionnaire, which must be filled out.
Our ability to draw practical conclusions about your present state of health and how to improve it depends, to a significant extent, on your ability to respond thoughtfully and accurately to your Health Questionnaire and those posed during your consultations with your provider. Careful consideration of your health questionnaire will help identify the underlying causes of illness and assist us in formulating a treatment plan and effectively using your scheduled consultation time.
Once you have booked your appointment and filled in the Health Questionnaire, your lab order will be sent to you through the Patient Portal. You can print this out and take it to the lab to have your blood drawn.
Getting Your Labs done ahead of your appointment
Labs must be drawn at least three weeks before your appointment, as it takes some time to get the lab results.
Please check with your insurance provider to verify who their preferred lab company is (usually Quest Diagnostics or LabCorp). You should also confirm that they will cover the cost of your lab tests. If you do not have insurance or your insurance will not cover this lab work, we have a special discount program through Quest and Lab Corp for patients without insurance or for patients whose insurance will not cover a portion of the lab. For new patients, the cost to you for your initial labs is a heavily discounted flat fee of $500 (if you are able to use Quest or get your labs done in our office) for what is usually about $3500 worth of labs. If you choose to self-pay for your labs, we can provide you with a superbill that you can present to your insurance company for reimbursement or minimally to count on your deductible.
We have a phlebotomist here in the clinic and if possible we would love for you to come into the clinic to get your labs drawn. You can also go to a Quest lab or Labcorp lab if the clinic is not as convenient.
Any previous labs or tests from another doctor/clinic you would like reviewed must be sent to us before your appointment. You can fax them to 972-695-4001 or send them to us as a PDF through the patient portal using the Messages tab.
Your Appointment
In this first 2-hour appointment, we will discuss the symptoms you are experiencing, what issues you would like to address, your health history, and any concerns you have. Your provider will review your test results with you in detail and explain how these relate to your symptoms. They will lay out a treatment plan and recommend the best treatment options, whether a medication or a supplement.
For those who are only looking to do our weight loss program or pellets, your appointments are much shorter.
Balancing hormones and other wellness recommendations takes time. Follow-up appointments will be needed to monitor and balance your levels and possibly address new issues. Shifts in diet, exercise, stress levels, and other lifestyle changes and factors can affect your hormones and your body. After your initial appointment, our providers like to see you every three months for your first year. After this, one can usually shift to every six months for continued maintenance.
Program and Fees
Your first appointment with Dr. Elwell will entail approximately 2 hours of quality time with her for a fee of $795, with the option of presenting your insurance for your lab fees. When you book your appointment, you will be asked to put down a $250 deposit, which goes toward the cost of your first appointment. Your $250 deposit will be applied to the $795 fee, leaving a balance of $545 due at your initial appointment. Each one-hour follow-up visit for the first year every three months is priced at $350. Biannual visits after the first year of treatment are priced at $450.
Nurse Practitioner Debbie Bluestein, Physician Assistant Kam Neal, and PA Jennifer Mata all follow the same protocols as Dr. Elwell but with the fee of $595 (for your first 2-hour appointment), with the option of presenting your insurance for your lab fees. When you book your appointment, a $250 deposit is required, which is applied towards the cost of your first appointment. Your $250 deposit will be applied to the $595 fee, leaving a balance of $345 due at your initial appointment. Each one-hour follow-up visit for the first year every three months is priced at $300. Biannual visits after the first year of treatment are priced at $400.
Depending on quantity and delivery method, hormones and supplements can range between $50 and $150 for a 30-day supply.
Insurance Coverage
Our providers are considered out-of-network providers. To get to the root cause of the problem and provide optimal treatment, we need more time with our patients, and more treatment options than insurance companies are willing to work with.
No matter what type of insurance you have, your insurance will frequently be able to cover all or a majority of your lab work. Labs written from our office are typically covered the same as if they were written by your regular doctor which is 90 to 100%. To find out what your insurance will cover, we encourage you to call the customer service number on the back of your insurance card and ask them.
If your insurance provider does not offer to cover a majority of the lab work or if it is being applied to your deductible, we recommend that you use our discount lab program that we offer through Quest Diagnostics and LabCorp.
If you have a PPO type of insurance plan, your insurance company may reimburse you for much of your office visit costs. This depends on your insurance company, what kind of plan you have, and your deductibles. You will pay for the appointments at the time of service, and we can provide you with the necessary papers (upon request) to file for reimbursement with your insurance provider.
If you are on an HMO plan, most likely, your insurance will not cover any of the costs of your office visits but will likely cover all or a majority of your lab work. Medicare coverage cannot be used.
When talking to your insurance customer service, tell them you’re scheduled to see an out-of-network provider who will be ordering very comprehensive lab work. Find out specifically what percentage of the lab costs they will pay. We offer a self-pay discount program for labs, a $600 flat fee for the complete set of labs to be drawn.
