We’re glad you’re here. At Forum Health Las Colinas, our team looks forward to partnering with you and supporting your journey toward better health.
Getting Started
When you first contact our office, you’ll speak with our New Patient Coordinator. This initial conversation is unhurried and designed to ensure you feel informed and supported.
Your New Patient Coordinator will:
-
Answer your questions in detail
-
Review our care model and treatment approach
-
Introduce you to our providers
-
Help determine whether our practice is a good fit for your needs
-
Schedule your initial appointment if you choose to proceed
The direct line to our New Patient Coordinator is 972-960-4800
Your First Appointment
All new patients begin with a two-hour initial appointment. Follow-up visits are typically one hour.
During your initial visit, your provider will:
- Review your symptoms and health concerns
- Discuss your health history in depth
- Go over your lab results in detail
- Explain how your results relate to your symptoms
- Create a personalized treatment plan, which may include medications, supplements, and lifestyle recommendations
Patients seeking care solely for weight-loss programs or pellet therapy will have shorter appointments.
Information We’ll Need from You
After you’ve scheduled your appointment, you’ll receive access to our Patient Portal, where you’ll find:
-
Consent forms
-
Your Health Questionnaire
Completing the Health Questionnaire thoroughly and thoughtfully is essential. It allows your provider to:
-
Identify underlying causes of symptoms
-
Make accurate clinical connections
-
Use your consultation time as effectively as possible
Lab Work: What to Expect
Once you complete your Health Questionnaire, your lab order will be sent through the Patient Portal.
Timing Matters
-
You must complete your labs at least three weeks before your initial appointment
-
This allows adequate time for results to be processed and reviewed
Where Labs Cab be Done
-
In our clinic (we have an on-site phlebotomist)
-
At a Quest Diagnostics
If our office is convenient, we strongly encourage on-site laboratory draws.
Insurance and Lab Coverage (Overview)
-
Most insurance plans cover all or a majority of lab work, even when ordered by an out-of-network provider
-
Labs are typically covered the same way they would be if ordered by your primary care physician
We recommend calling the customer service number on the back of your insurance card to:
-
Confirm your preferred lab provider (usually Quest)
-
Verify coverage details for lab testing
If insurance does not cover your labs or applies them to your deductible, we offer discounted lab programs through Quest Diagnostics.
Sending Prior Records
If you’d like previous labs or medical tests reviewed before your visit, please send them in advance.
You may:
-
Fax records to 972-695-4001
-
Upload PDFs through the Patient Portal using the Messages tab
Ongoing Care & Follow-up

Because diet, stress, exercise, and lifestyle changes can affect progress, follow-up visits are important for:
-
Monitoring results
-
Adjusting treatment plans
-
Addressing new or evolving concerns
Typical follow-up cadence:
-
Every three months during the first year
-
Every six months thereafter for maintenance
Insurance Considerations
-
Our providers are out-of-network
-
This allows for longer visits, deeper investigation, and broader treatment options
Important notes:
-
PPO plans may reimburse a portion of visit costs (varies by plan)
-
HMO plans typically do not cover office visits, but often cover lab work
-
Medicare cannot be used
-
Payment is due at the time of service
We can provide documentation (upon request) for insurance reimbursement submissions.
