About Dr. Kyler Elwell
Dr. Kyler Elwell-Silver is a board-certified physician passionate about medicine and deeply committed to patient care. After years of practicing conventional medicine in a successful career as an obstetrician-gynecologist, Dr. Elwell reached a point where she felt the need to explore a different approach and decided to transition to functional medicine. The increasing number of inquiries about functional medicine and the desire to provide comprehensive care beyond the limitations of 15-minute appointments prompted her to do some soul-searching and make a change.
Dr. Elwell found that patients wanted to understand the reasons behind their illnesses and desired solutions beyond symptom management. They sought answers to the “why” behind their health issues and longed for sustainable solutions. Inspired by this desire for comprehensive care, she embarked on a new path in functional medicine, eager to provide patients with the in-depth understanding and personalized treatment plans they desired.
Dr. Elwell completed her undergraduate and master’s degrees at the University of Nevada, Las Vegas, before pursuing her medical degree at the University of Texas Medical Branch in Galveston. She then completed her residency in obstetrics and gynecology at UT Southwestern, solidifying her foundation in women’s health. Dr. Elwell most recently became board-certified in obesity medicine, further enhancing her ability to address the complex relationship between hormones, metabolism, and overall health. Creating an effective, science-based approach of diet, exercise, stress management, and appropriate hormone adjustments often does the following:

- improve memory and concentration
- reduce heart disease and diabetes risk
- improve energy
- control weight gain and lessen abdominal fat

She realized that her favorite part of being a physician was the deep connections she formed with her patients. Physician Assistant Kam Neal has loved medicine her entire life.
She grew up in Colorado, and as a child, she remembers being fascinated with investigating how things worked. Kam attended Southern Nazarene University in Bethany, Oklahoma, where she received a bachelor of science in biology. Following that, she completed the physician assistant program and earned a master’s degree in health services from the University of Oklahoma Health Sciences Center.
Kam is a certified Physician Assistant licensed by the Texas Medical Board.
In the traditional medical model, Kam felt frustrated working in a high-volume practice. She had barely enough time to refill medications and then didn’t often see her patients feel any better.
In addition, her own experience with hormone imbalances after having children and being told “everything is normal” fueled those feelings. This led her to functional medicine and an optimal wellness model.
Kam’s desire to treat the whole patient and not just the symptoms, along with her passion for teaching, is evident: She loves working with each patient to find the root cause of their illness while teaching them about their health so they feel informed and empowered.
Nurse Practitioner Debbie Bluestein works with both women and men using Bioidentical Hormones.
Debbie is passionate about helping people feel their best, achieve optimal health, and prevent illness and disease. She believes in using a very individualized approach to achieve optimal hormone balance for each patient and helping patients with chronic medical issues.
Debbie listens closely to her patients and involves them in decision-making regarding their bodies and health. She collaborates with the practice when planning a comprehensive program for clients.
As a Nurse Practitioner since 1998, Debbie has had opportunities for extensive study and training in Bioidentical Hormone Replacement Therapy and optimization. She uses a functional medicine approach to anti-aging, using hormones, diet, exercise, nutraceuticals, and supplements.
Debra worked in Family Practice during the early part of her career and has been working with Bioidentical Hormone Replacement Therapy since 2011.
Jennifer Mata, PA-C
Jennifer Mata is a highly dedicated and compassionate Physician Assistant at Forum Health. She specializes in holistic and preventative care.
Jennifer has extensive skills and experience in: Bioidentical Hormone Replacement Therapy, Sleep Care Management, Weight Loss Management, and Primary Care.
With a philosophy centered on natural approaches and patient education, Jennifer creates an open dialogue with her patients to ensure they are active participants in their health journey.
Her professional journey includes significant roles at various healthcare institutions. At Star Sleep and Wellness in Irving, TX, Jennifer focused on evaluating and treating chronic sleep conditions. During her tenure at Healthcare Associates of Texas in Irving, TX, she played a pivotal role in the Wellness Clinic Division, focusing on weight management, diabetic education, nutritional management, treatment of sleep issues, and hormone management.
Jennifer’s commitment to her patients is deeply personal. She was a patient of this very clinic for over a year, looking for answers to some of her health issues.
Jennifer’s approach begins with really listening to her patients to gain an understanding of what they have been experiencing and forming a partnership with every patient. She believes in setting realistic goals and expectations, understanding that each patient’s needs are unique. As she says, “I’m your advocate, your cheerleader, and we’ll work together to make you feel better overall.”
Outside of her professional life, Jennifer is a lifelong learner, constantly updating her knowledge to better serve her patients. Nurse Practitioner Jordyn Ghanem
Jordyn is a dedicated Nurse Practitioner who brings both clinical expertise and heartfelt compassion to our practice. She earned her Master of Science in Nursing (MSN) Magna Cum Laude from Texas Woman’s University, preceded by an accelerated Bachelor of Science in Nursing (BSN) from Texas Christian University and a Bachelor of Science in Family and Child Sciences from Florida State University. Jordyn holds an Advanced Practice license in the state of Texas and is nationally board-certified as a Family Nurse Practitioner. Originally from Boca Raton, she now lives and practices in Dallas, TX.
Over the years, she has worked across multiple specialties including pediatric cardiology, plastic surgery, cosmetic dermatology and general surgery gaining a broad and holistic understanding of patient care. However, it was in integrative medicine that Jordyn found a deeper purpose, one that goes beyond symptom management to uncover and address the root causes of illness.
Jordyn is certified in Critical Care Nursing, Pediatric Advanced Life Support, and Advanced Cardiovascular Life Support, and is a Board-Certified Family Nurse Practitioner. Her broad clinical training includes primary care, behavioral health, pediatrics, maternity, and critical care, with rotations at several leading healthcare systems across Texas.
Most recently, Jordyn served as an OR Registered Nurse/Circulator at Baylor Scott & White Surgicare, where she provided comprehensive care across a wide range of surgical specialties including Orthopedics, ENT, Gynecology, and General Surgery. She also worked as a Nurse Practitioner injector at House of Her Aesthetics, offering mobile aesthetic services such as botulinum toxin and dermal filler injections tailored to each client’s comfort and convenience.
Her previous experience includes roles at several top-tier aesthetic and plastic surgery practices throughout the Dallas area. Jordyn not only brings her incredible training, skills and experience to Forum Health Las Colinas, but she has also allowed us to now be able offer a range of Aesthetics Services here at the clinic.

The safety of our staff and patients is a top priority for all of us at our office. We offer both virtual appointments and in-clinic appointments, whichever you prefer. Know that we maintain the highest standards when it comes to the safety of our patients. We sanitize rooms between patients and also sanitize shared surfaces, such as door handles, throughout the day.
Should you choose to have a telemedicine appointment, you will still be able to see your provider, just as if she were right in front of you in the clinic. Additionally, we have mobile phlebotomy services available if you do not want to go to a public lab.
